What Is Periorificial Dermatitis?

Periorificial dermatitis (also called perioral dermatitis when it’s localized to the mouth) presents as red, inflamed bumps and patches of irritated skin. The rash can resemble acne or rosacea but typically spares the skin immediately adjacent to the lips. In some cases, mild scaling or burning may be present.

This condition most frequently affects:

  • Women aged 20–45

  • Children (often around the nose and eyes)

  • Individuals using topical steroids

Common Triggers

The exact cause of periorificial dermatitis is unknown, but several triggers have been identified:

  • Topical corticosteroids (including over-the-counter hydrocortisone)

  • Inhaled corticosteroids (especially if used without a spacer)

  • Cosmetics and moisturizers

  • Fluoridated toothpaste

  • Hormonal changes

  • Microbial imbalance or overgrowth (e.g., fusiform bacteria or candida)

Symptoms

  • Red, bumpy rash around the mouth, nose, or eyes

  • Mild burning or itching

  • Absence of comedones (blackheads or whiteheads), distinguishing it from acne
